SERIKALI
ya awamu ya tano chini ya Rais Dk. John Magufuli imewaahidi Watanzania
kuboresha sekta ya afya. Hili ni jambo jema.
Imeahidi
kujenga hospitali za wilaya, mikoa na hospitali za rufaa pamoja na kutekeleza
Sera ya Afya inayosisitiza kwamba kila kijiji kiwe na zahanati na kkila kata
iwe na kituo cha afya ili kusogeza karibu huduma za afya kwa jamii.
Licha
ya kuahidi pia upatikanaji wa dawa, lakini bado haijazungumzia mikakati yake ya
kuhakikisha wanakuwepo watumishi wa kutosha wa afya, hususan madaktari.
Katika
kipindi cha takriban miaka 10 iliyopita Tanzania imeshuhudia wimbi la madaktari
likitimka nchini kwenda katika mataifa mengine kusaka mishahara minono.
Hali
hiyo inatokana na mazingira duni ya kufanyia kazi hapa nchini pamoja na
mishaara midogo licha ya mzigo mkubwa wa kuhudumia wagonjwa wengi huku mamlaka
husika zikishindwa kusikiliza kilio cha wataalamu hao wa tiba ambao wakati
mwingine wanalazimika kugoma ili kuishinikiza serikali.
Tumeshuhudia
mara kadhaa migomo hiyo ya madaktari ikileta athari kubwa kwa wananchi, ikiwemo
vifo vya wagonjwa wasio na hatia, lakini badala ya kutatua matatizo yaliyopo,
serikali imekuwa ikiwafukuza kazi na kuwasimamisha wengine kwa migomo hiyo.
Matokeo
yake Tanzania sasa imekuwa miongoni mwa nchi tisa zilizoko Kusini mwa Jangwa la
Sahara ambazo zinapoteza mabilioni ya fedha kuwasomesha madaktari, lakini
baadaye wanatimikia nje kusaka maisha mazuri.
Uamuzi
wa kufukuza kazi au kuwasimamisha madaktari haujaleta nafuu kwa taifa, badala
yake umeleta hasara kubwa huku ukizinufaisha nchi kama Botswana, Namibia na
Zambia, ambako inaelezwa kwamba maslahi ni mazuri zaidi na ndiko wataalamu wetu
wanakokimbilia.
Utafiti
uliofanywa na Edward Millas wa Taasisi ya Global Health ya Chuo Kikuu cha
Ottawa, unaonyesha kwamba Tanzania ni moja kati ya nchi tisa za Kusini mwa
Jangwa la Sahara ambazo zinapoteza mabilioni ya fedha kwa kuwasomesha madaktari
lakini wanatimka kwenda nchi nyingine.
Mbali ya Tanzania, nchi nyingine zinazoathirika na kuondoka kwa
madaktari wake ni Ethiopia, Kenya, Malawi,
Nigeria, Afrika Kusini, Uganda, Zambia na Zimbabwe.
Kwa
mujibu wa utafiti huo, Afrika Kusini na Zimbabwe ndizo zinazoongoza kupoteza
fedha nyingi kusomesha madaktari hao, ambao hatimaye wanatimkia nje, huku
Australia, Canada, Uingereza na Marekani zikinufaika zaidi kwa kuwaajiri
madaktari hao.
Utafiti huo unaeleza takwimu za
namna nchi husika zinavyotumia fedha kuwasomesha madaktari na jinsi nchi
nyingine zinavyookoa fedha hizo kwa kuwaajiri madaktari hao badala ya
kuwasomesha wao.
Matokeo ya utafiti huo
yanaonyesha kwamba, serikali katika nchi hizo zinatumia kati ya Dola za
Marekani 21,000 (kwa nchi ya Uganda) hadi Dola 59,000 (Afrika Kusini)
kumsomesha daktari mmoja, ambapo kwa nchi tisa zilizofanyiwa utafiti,
inaonekana zilitumia kiasi cha Dola 2 bilioni (takriban Sh 4.4 trilioni)
kuwasomesha madaktari wao na hazikuweza kuwatumia.
Lakini Uingereza inaokoa karibu
Dola 2.7 bilioni (Sh 6 trilioni) huku Marekani ikiokoa Dola 846 milioni (Sh 1.9
bilioni) kwa kutowasomesha madaktari wake na kuajiri wale kutoka nchi
zinazoendelea.
Itakumbukwa
kwamba, mara baada ya mgomo wa Januari na Juni 2012, madaktari 80, wakiwemo
madaktari bingwa, kati ya 200 waliofukuzwa kazi
kutokana na kushiriki mgomo waliondoka nchini kwenda Kusini mwa Afrika kati ya
mwezi Januari na Juni.
Hata
hivyo, kuondoka kwa madaktari hao ambao wanatafuta maslahi zaidi, kunaelezwa
kumechangiwa na uzembe wa Serikali katika kutatua kero na kuboresha maslahi yao
kwa miaka mingi sasa.
Taarifa
zinasema, madaktari wengi wanakimbilia Botswana kwa sababu nchi hiyo inawalipa
vizuri zaidi, sambamba na walimu, na kwamba ndiyo inayoongoza kwa kuwa na
madaktari wengi kutoka Tanzania pamoja na wataalam wa fani nyinginezo, hasa za
uhandisi.
Takwimu
za Shirika la Afya Duniani za mwaka 2006, zinaonyesha kuwa, uwiano wa daktari
na wagonjwa kwa nchi ya Botswana ni 1: 2,500, wakati ambapo Tanzania, ambayo
inaendelea kuwapoteza madaktari kila siku, uwiano wake ulikuwa 1: 50,000.
Baada
ya mgomo wa Juni 23, 2012 Serikali ilitangaza kuwafukuza kazi madaktari zaidi
ya 200 waliodaiwa kushiriki mgomo kutoka Hospitali ya Rufaa Mbeya, Hospitali ya
Mkoa wa Dodoma, Hospitali ya Rufaa Bugando na Hospitali ya Taifa Muhimbili.
Hata
hivyo, wengi kati ya waliofukuzwa ni wale ambao walikuwa mafunzoni (interns),
huku madaktari bingwa wakisimamishwa kazi kwa muda usiojulikana, hali ambayo iliwafanya
wengi wao kutafuta ajira nje ya nchi.
Januari
2012, uongozi wa Hospitali ya Taifa Muhimbili (MNH) ulitangaza
kuwafukuza kazi madaktari 229 waliokuwa katika mafunzo kwa njia ya vitendo
kutokana na mgomo wa siku nne wakishinikiza kulipwa posho zao za mwezi mmoja,
takriban Shs. 176 milioni.
Kuondoka kwa madaktari hao na
watumishi wa afya kutoka katika nchi zinazoendelea kama Tanzania kukichangiwa
na migomo kunazorotesha mfumo mzima wa afya, hivyo kuwa tishio katika kufikia
Malengo ya Endelevu ya Dunia.
Mwaka 2010, Jukwaa la Afya la
Dunia (World Health Assembly) liliridhia azimio la kwanza la kimataifa katika
kuajiri wataalam wa afya kwa kutambua matatizo yanayosababishwa na kuhama kwa
madaktari kutoka nchi maskini kwenda nchi tajiri, ambazo hatimaye ndizo
zinazotoa misaada katika sekta ya afya.
Azimio hilo ni muhimu kwa nchi
za Kusini mwa Jangwa la Sahara lenye uhaba mkubwa wa watendaji katika sekta
hiyo, lakini wachunguzi wa mambo wanasema, kitendo cha serikali kupuuza madai
ya madaktari na kusababisha migomo kinarudisha nyuma azma ya kutatua matatizo
hayo.
Pengine ni wakati muafaka sasa
kwa serikali kuboresha mazingira ya kufanyia kazi pamoja na maslahi ya
watumishi wa sekta ya afya, ambayo siyo tu yatawavutia wataalamu walioko nchini
kufanya kazi, bali pia kuwavutia wale waliotimka kurejea na kuwahudumia
Watanzania wenzao.
